It’s because she’s a main character and an executive producer of the show. Kendall and Kylie aren’t that’s why they don’t appear that much. Kim and Khloe gets private when it comes to True or Kanye too, but they share work and other parts of their love life while Kourtney doesn’t share about Travis Barker or whatever she’s working on (Poosh)
Well she a different contract than kylie, Kourtney was able to leave the show in season 17 but she still decided to stay until season 20.

What's sad is the transference of insecurity . one boy made Kylie feel insecure about her lips, and that made her feel that thin lips were undesirable, so she changed it and she didn't obviously realise it at that time because she was doing that for personal reasons but that impacted beauty standards at that time which resulted in thousands of young impressionable girls feeling like their thin lips were undesirable and made them feel insecure.

She's also had work done (allegedly), but she's the only one who went moderate with it because she's a model and needs to fit most of the designers standards (thin, almost no curves, chiseled but yet soft face, etc) if she went hammed with the surgeries I don't think she would've been able to work with couture high-end designers like she does now, but more commercial modeling instead. I think she decided to keep her surgeries more on the lowkey side.
