Malekith and the dark elves have been stated to be born before the beginning of the universe and lived in darkness. That was their whole objective in the film, to use the reality stone to warp the universe back to it's original state of darkness :/
We know that the dark elves existed before the universe. But I suppose that new dark have come into existence since that time as well. Do we know that Malekith was indeed one of the dark elves that existed before the universe? Or has he come into existence since?
If he, like others of his race, existed before the universe, then he should definitely be higher in this list (higher than the Grandmaster and Collector even I'd say). Otherwise it's hard to tell exactly how old he is, but it still seems likely that he should be higher in this list, but it isn't really possible to tell without any exact information.
I couldn't find anything conclusive on whether Malekith is indeed one of those dark elves that existed before the universe or how old he is. But maybe you have sources that I don't know of?

Like Galactus, other beings such as Mephisto may have been creations of the entity that became the six singularities in the current Marvel 616 universe. Mephisto explained this to Thanos I believe from one of the stories before the 6 issue Infinity Gauntlet series. A few beings that claim to be older than time itself were beings created by that entity. Like most Marvel origins, the information is complex and very deep.
