이번 곡은 감각적인 아티스트 ‘서사무엘’이 가창 주자로 참여하였다. 서사무엘은 작사, 작곡, 편곡에서부터 노래와 랩까지 직접 작업을 할 정도로 유명 아티스트사이에서 ‘천재 아티스트’라는 수식어로 주목을 받아왔다. 2015년 발매한 그의 첫 번째 정규앨범 ‘FRAMEWORKS’로 2016년 한국대중음악상 최우수 R&B/Soul 앨범상을 수상하였고, 그만의 음악적 색깔과 입지를 굳혀 대중에게 그 가치를 인정받았다.
드라마 ‘의사요한’은 통증의학과 의사들의 이야기를 그린 휴먼 메디컬 드라마로 인간의 존엄사에 대한 내용을 담고 있다. 존엄사는 임종 단계에 있는 환자가 연명치료를 중단하고 자연적 죽음을 받아들이는 것으로 OST Part.4 ‘Pain or Death’는 바로 이러한 주제를 관통하는 내용을 담고 있는 곡이다.
고통속에서 하루하루 생명을 이어 나갈 것인지 죽음을 택할 것인지, 아니면 의사의 입장에서 고통을 연장할 것인지 고통을 해결할 것인지에 대한 진지함을 차가운 사운드와 담담한 서사무엘의 보컬로 표현하였다.

hotel del luna is the biggest hit for kdrama in 2019 but for me, i can't forget doctor john. like whenever i listen to the songs it makes me feel like watching it again😭

For Ji-Sung, I actually prefer him in melodramatic comedies . . . like 'Kill Me Heal Me' and 'The Entertainer' . . . he has the persona to be humorous without being over dramatic, yet remain mysterious at the same time. I'm going to give 'Doctor John' a second chance, but I remember not enjoying 'Secret' very much.
Dark and deep melodramas are very hard to pull off . . . so far the Taiwanese drama 'Mars' with Vic Zhou is the one that tops that genre for me. A little back story, Vic Zhou, in reality, struggled with depression in order to express those emotions on screen.

I have watched this drama two times already. I just can't get enough of it. The scenes, settings, acting, ost and all. It's just so good. It made me cry, happy, wonder, amaze, confuse and all. Back then, I always complain of the pain and hassle of living with sadness day by day but watching this drama, I realized that having to feel nothing is a lot harder. It's like breathing yet not living at all. Thanks, Ji-Sung.

What I feel inside is indescribable. This actor's talent is like an inexhaustible supply of talent. A bottomless pit that is always full of beauty, charisma, and everything in fact. Jisung has the gift of moving me with his eyes. He can express so much, and it's just wonderful to watch.
Their chemistry is unparalleled. It pierces my heart with a radiant, warm arrow. Se-Young is beautiful, she has a sweet presence, and shows through her character a strong character. She doesn't give up at the first defeat, and constantly tries to break Yo-han's shell. Despite the fact that he pushes her away, she continues. And I think that's beautiful. She wants to get into his world, to discover him, to know more about him so that she can understand the man she loves.
Because this disease that is eating away at him seems to be terribly painful. He doesn't feel anything, but his heart suffers.
Really, I regret so much to have discovered this actor only this year. In Kill me heal me he captivated me.
I think I missed his 20 years of career and it makes me a little sad but still, I could discover him and it's the best feeling to see him play 😍
